Clock reference to anatomic position

+

Spell out o’clock when anatomic position is described in terms of clock-face orientation as seen by the dictator.

+

These are not expressions of time; do not transcribe them as such.

+

These expressions form a compound modifier preceding and modifying the word position and therefore are hyphenated.

+

EXAMPLE:

+ The incision was made at the 6-o’clock position.

+

If the word position is not dictated but is clearly implied (i.e., it is obvious that the reference is to an anatomic position in terms of clock face orientation), then transcribe the full expression to avoid confusion.

+

EXAMPLE:

+ Dictated: The incision was made at 6-o’clock.

+ Transcribed: The incision was made at the 6-o’clock position.

+

When the position is expressed as a partial hour, omit the word o’clock, even if it is dictated.

+

EXAMPLE:

+ Dictated: The incision was made at the 12:30-o’clock position.

+ Transcribed: The incision was made at the 12:30 position.
