Surgical wires, pins and screws

When more than one dimension is dictated to identify the size of a screw or a pin (thread diameter and thread length, for example), these dimensions are expressed as fractions, with the first size (diameter) over the second (length). Be careful to express diameter as a decimal when indicated.

EXAMPLE:

Dictated: A six five sixteen cancellous bone screw.

Transcribed: A 6.5/16 cancellous bone screw or A 6.5/16 mm cancellous bone screw.>

Kirschner Wire

Kirschner wires, or K-wires, are sterilized, sharpened, smooth stainless steel pins used in orthopaedics, plastic surgery, and other types of medical and veterinary surgery. They come in different sizes and are used to hold bone fragments together (pin fixation) or to provide an anchor for skeletal traction. The pins are often driven into the bone through the skin (percutaneous pin fixation) using a power or hand drill.

This type of wire presents a big exception: they are measured in portions of inches, but are written using decimals expressed in thousandths. Typically, the unit of measure it not expressed.

Include the zero both before and after the decimal point. Most physicians will not dictate either one and it’s important to remember that K-wire measurements are in thousandths and therefore require the zero after the decimal point.

EXAMPLE:

Dictated: A two eight K wire and a six two K wire.

Transcription: A 0.028 K-wire and a 0.062 K-wire.

NOT

A .028 K-wire and a .062 K-wire or

A .28 K-wire and a .62 K-wire or

A 28 K-wire and a 62 K-wire or

A 0.028 inch K-wire and a 0.062 inch K-wire or

A 0.028 mm K-wire and a 0.062 mm K-wire or

any variation of these negative examples.
